Unique° Staff Applications

Please fill out the following form and submit before 12/9/2018 in order to become a staff member at Unique°. Answer all questions honestly and make sure you are expressing your own thoughts/opinions and not the thoughts of someone else. If you are accepted, you will be ranked to Assistant Host and you will then be required to attend 1 of the 3 training sessions hosted.
